Important Announcement

 

Dear Patients and Community Members,

As I arrive at my retirement date and the closure of my medical practice, I would like to express my appreciation for your support and understanding during this period of transition. My final day of patient appointments was on Wednesday, May 29th, and we packed and moved out of the office on Thursday and Friday of the same week. Your health and well-being have always been my utmost concern, and I want to assure you that my staff and I have diligently followed up on any remaining referrals, tests, and prescriptions that I have authorized. If you undergo any tests that I had previously recommended, kindly inform the testing facility about your new primary care physician. This is an essential step to ensure that your results are promptly forwarded to your new physician, even if your first appointment is not scheduled for some time.

If you have not already done so, I strongly urge you to find a new doctor to ensure the continuity of your health care. Begin by asking for recommendations from friends, family, or your health insurance plan. To help you determine if a physician may be a good fit, consider calling or stopping by the provider’s office and speaking with their staff. Most importantly, schedule a new patient appointment with a primary care physician. I have included several resources in the links above to help you with this.  

I want to extend my heartfelt thanks to all of you who have scheduled final appointments with me and to those who have reached out to my staff to discuss the transition of your care. Please take a moment to read my care suggestions including accessing emergency, urgent, telehealth, and primary care services, as well as detailed information on how to request your medical records. I hope you will find these resources useful as you manage this transition, especially if you didn’t have a chance to connect with us before my retirement.

On a personal note, it was difficult for me to decide to retire, and I have been deeply touched by the kindness and appreciation expressed to me as I approached this milestone. Every card, gift, treat, and flower has touched my heart. I will always treasure the memory of the final few months of my medical practice when every available surface was beautifully adorned with flowers gifted to me by patients.

This medical practice, along with the patients and staff within it, is the heart of my life's work. I am immensely grateful for the enduring love, wonder, and zeal I feel for my profession, this community, and for all of you.
